One of the first steps towards gaining a driver's licence is getting the medical certificate for licence. It is crucial in order to become a driver. When a repeated health examination must be done?

Drivers of such categories as AM, A1, A, B1, BE and B are obliged to do the mandatory health examination after every 10 years but drivers after 60 - after every 3 years. But drivers of such categories as C1, C, C1E, D, D1E and DE must do the checkup after every 5 years and if they are over 60 - after every 3 years.

Medical commission may also carry out an early health inspection based on the request of the driver himself/herself. An early heath examination may also have to be done based on the directions of police in case if the driver's licence has been revoked for driving under the influence of alcohol or drugs.

In case a driver has serious health issues related to the statutory medical contraindications when driving a vehicle and the condition is likely to deteriorate, it may be stipulated that a vehicle can be driven only after regular health checkups. For example, persons with mental disorders are allowed to drive a car only by the approval of a psychiatrist but not longer than 1 year, which in this case means mandatory checkups at least once a year. Depending on the health condition of the particular driver, the frequency of those examinations may vary from person to person.

What health problems come in the way of getting a licence?

Overall, there are 8 groups of diseases, and in case the future or current driver's health condition falls into one of those categories, a medical certificate may not be issued.

The first medical contraindication for driving a vehicle is neurological diseases and obstructive sleep apnea syndrome. If the central or peripheral nervous system is affected as a result of the neurological diseases or disorders that have caused sensory or motor impairment, affecting coordination and the sense of coordination and balance, the medical commission must evaluate the location, extent and degree of the particular disorder. Not in all cases driving a vehicle is strictly prohibited, but the driver may be subject to certain restrictions, for example, a specific modification type of the vehicle may be indicated. However, if such manifestations of the disease as central and peripheral paresis, ataxia, impaired locomotor or upper nervous system function, etc., may have a toll on the safety of other road users, the option to drive a vehicle may be fully ruled out.

The second group of diseases is mental disorders. Such severe behavioural problems related to age or personality disorders that affect the ability to judge, adapt and react, are considered to be dangerous for road traffic. Mental retardation severe mental disorders due to endogenous or exogenous changes are also a part of this group.

The third medical contraindication is the addiction of alcohol or drugs. The only exception when driving may be permitted and the medical certificate issued, is in case the driver has a proof of an one-year abstinence period.

The fourth group of contraindications is internal diseases. Driving a car may be prohibited in case of any internal disease if it is said that the development of the disease may lead to brain disorders or endanger the safety of other road users.

Certain restrictions also apply to persons with visual impairment. The medical certificate is not issued to persons with a binocular visual acuity of less than 0,5 when looking with both eyes at the same time, or with a visual acuity of less than 0,5 in the single eye that sees. Also other restrictions apply for visual impairment, and the representatives of the medical commision must evaluate them.

The seventh group is hearing impairment. There are various contraindications identified in this group: chronic unilateral or bilateral purulent otitis media with granulation, polyps cholesteatoma; complications after fibula or a cyst; not hearing a whispering voice within 1 metre of distance or not hearing a person talk with each ear within a distance of 2 metres. Driving a vehicle may be permitted after evaluating the severity of the disorders.

As a result of a trauma, driving a car may also be prohibited. Contraindications are related to hand, leg or palm deformation and injury which interfere with the movement.

Finally, there are a number of cardiovascular diseases identified as a result of which issuing a medical certificate and driving a car is strictly prohibited. Those are, for example, such diseases as tachyarrhythmias, bradyarrhythmias, acute coronary syndrome, precutaneous coronary intervention, stroke, transient ischemic attack, heart failure, etc. Driving a car may be allowed in case a medical institution approves it and the driver has regular medical checkups.
